CHIRP    http://www.chirp.co.uk Report safety related incidents anonymously A charitable institution, CHIRP stands for Confidential Hazardous Incident Reporting Programme, and was pioneered from 1982 in the aviation industry. CHIRP encourages reporting of incidents by ensuring that the individual remains anonymous and doesnot suffer any consequences from their employers. CHIRP then represents any safety problems it finds as a result of reports to the institutions involved, encouraging them to improve practices and helping to improve safety all round.
