The Marine Accident Investigation Branch (MAIB) looks into incidents at sea and provides recommendations for safer practices. An incident reporting form is available for download in acrobat (53 kB pdf) format on the site. There is also a summary of current investigations, regular safety digests and safety bulletins describing particular incidents.
